PURPOSE AND EFFECT: The Board proposes the rule amendment to update the language to allow for electronic notification of change of address.

64B1-7.004 Notice to the Agency of Mailing Address and Place of Practice of Licensee.

(1) No change.

(2) Any time that the current mailing address or place of practice of any licensee changes, written notification of the change shall be provided to the Department within 10 days of the change. Written notice should be sent to the following address: Board of Acupuncture, 4052 Bald Cypress Way, Bin C-06, Tallahassee, Florida 32399-3256.  Electronic notification to [e-mail], is also acceptable, however, it is the responsibility of the licensee to ensure that the electronic notification was received by the Department.
