    PURPOSE AND EFFECT: To update the biennial renewal licensing requirements and forms DH-MQA 1229 and DH-MQA 1231 to clarify podiatric x-ray assistant title and background screening requirements, add optometrists to list of dispensers of medicine drugs, add pain management clinic exemptions to renewal schedule as required by recently enacted legislation, add DEA numbers to information required from controlled substance prescribers and dispensers, amend dates and delete language relating to physical addresses.
