REQUEST FOR PROPOSALS
Competitive sealed proposals will be received by the Area Agency on Aging for North Florida until 3:00 p.m. on September 2, 2016 for the designation of Community Care for the Elderly Lead Agency in Franklin County. The Community Care for the Elderly Lead Agency designation includes the provision of an array of home and community based services to frail older persons. The Community Care for the Elderly program is administered pursuant to provisions of Chapter 430, F.S. The proposal package and application instructions will be available from the Area Agency on Aging for North Florida on July 15, 2016 or thereafter at the correspondence address listed below, or via the agency’s website at www.aaanf.org. The Area Agency on Aging for North Florida reserves the right to reject any and all proposals.
Every entity that intends to submit a proposal must deliver the required Notice of Intent Form (Appendix III) to the Area Agency on Aging for North Florida, Inc. at the correspondence address provided below by 5:00 p.m. on Friday, August 19, 2016. In the event that less than two Notices of Intent are received, the Area Agency on Aging for North Florida may institute the Exceptional Purchase (as defined in Section 287.012, Florida Statutes) provisions provided for by Chapter 287, Florida Statutes for a single source contract. Should this occur, no response to Request for Proposals shall be needed and the Agency will proceed under the single source provisions of Section 287.057, Florida Statutes.
Any entity that protests the intended decision pertaining to the Exceptional Purchase shall be required to post, at the time of filing a formal written protest, a bond in the amount equal to 1 percent of the estimated contract amount for the Exceptional Purchase pursuant to Section 287.042, Florida Statutes.

A Bidders Conference will be conducted concerning this Request for Proposals at 11:00 a.m. on July 29, 2016. Interested parties must attend the Bidders Conference at the following location:
