NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 39, No. 77, April 19, 2013 issue of the Florida Administrative Register.
The change is in response to comments stated by the Joint Administrative Procedures Committee in a letter dated April 26, 2013 and concerns by the Board stated at its meeting on April 26, 2013.
The change is as follows:
(1) All applicants for licensure pursuant to Chapter 490, F.S., shall complete and submit form DH-MQA 1187, (Revised 04/13 07/12), “Application for Psychologist Licensure,” which is incorporated herein by reference and which may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-02059, the Board office, or at http://www.doh.state.fl.us/mqa/psychology
