NOTICE IS HEREBY GIVEN that on August 02, 2017, the Board of Optometry, received a petition for permanent variance filed by Michael Siebert, from subsections 64B13-5.001(2) and (7), F.A.C., regarding the requirement that as a condition of license renewal each licensed practitioner shall complete 6-hours of transcript quality CE and a 2-hour course relating to prevention of medical errors.
