NOTICE IS HEREBY GIVEN THAT on July 7, 2010, the Board of Optometry received a petition for waiver or variance filed by Thomas R. Karkkainen, O.D., M.S., from Rule 64B13-4.001, F.A.C., with regard to the licensure requirement of Rule 64B13-4.001, F.A.C., that applicants submit scores on the national licensing exam taken no longer than 5 years prior to application.
