NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 42 No. 103, May 26, 2016 issue of the Florida Administrative Register.
THE TEXT OF THE PROPOSED RULE WILL NOW READ:
64B9-6.003 Reactivation of Inactive License.
(1) An inactive license may be reactivated upon submitting a completed Reactivation Application, form number DH-MQOA 1200, 6/13, hereby incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref_____ or from the Board office or on the Board's website: www.FloridasNursing.gov.
(2) No change.

The effective date at the bottom of the application page has been changed from 6/13 to 6/16.
On the third unnumbered page, number 6 has been removed. It is no longer statutorily authorized.
On the fourth unnumbered page, a paragraph is being added to explain the necessity of providing a social security number.
The sixth unnumbered page will reflect an updated version of the rule. The rule is attached to the application to provide the applicant with the actual rule provisions for reactivation of an inactive license.
