NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 49 No. 140, July 20, 2023 issue of the Florida Administrative Register. Incorporated forms have been amended to: 1) add a field for email address on the DS-DE 9; 2) move the field for phonetic spelling from the DS-DE 9 to the back of each of the oath forms by limiting the space for listing fines, fees, and penalties; and 3) adding the phonetic spelling instructions to each of the oath forms as the final page.
