NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 35 No. 25, June 26, 2009 issue of the Florida Administrative Weekly.

Section (1) now reads: Requests for Institutional Review Board review shall be submitted electronically using the Department's electronic system. Payment shall be remitted to the Office of Public Health Research, Department of Health, 4052 Bald Cypress Way, Bin #A-24, Tallahassee, Florida 32399, [phone]

Section (9) now reads: Fees are nonrefundable, except if a fee is paid when none is due.
