    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 40, No. 78, April 22, 2014 issue of the Florida Administrative Register.

    The changes are in response to comments submitted by the Joint Administrative Procedures Committee in a letter dated April 25, 2014. The changes are as follows:

     

    Changes to the rule text are:

     

    The revised date in (1)(c) shall now read:  (Revised 07/14 7/12)

     

    Law Implemented shall now read: 456.015, 456.0635 FS.

     

    Changes to the incorporated application are:

     

    Page 3, paragraph under 21 shall now read:  For Questions 19-21:  The act of knowingly giving false information in the course of applying for or obtaining a license from the department, or any board thereunder, with intent to mislead a public servant in the performance of his or her official duties, or the act of attempting to obtain or obtaining a license from the department, or any board thereunder, to practice a profession by knowingly misleading statements or knowing misrepresentations constitutes a felony of the third degree, punishable as provided in s.775.082, s. 775.083, s. 775.084.

     

    Page 6, the 5th and 6th paragraphs, 33 Statement of Applicant, shall now read:

     

    I have carefully read the questions in the foregoing application and have answered them completely, without reservations of any kind, and I declare that my answers and all statements made by me herein are true and correct.  Should I furnish any false information in this application, or in any supporting documentation, I hereby agree that such an act constitutes cause for denial, suspension, or revocation of my limited license to practice psychology under Chapter 490, Florida statutes, Chapter 456, Part II, Florida Statutes, and Title 64B19, Florida Administrative Code, in the State of Florida.

     

    I further state that I have received, read and understand Chapters 456 and 490, Florida Statutes, and Title 64B19, Florida Administrative Code, and acknowledge that I must abide by them.
