REQUEST FOR INFORMATION
South Florida Community Care Network, LLC d/b/a Community Care Plan (CCP) is issuing a Request for Information (RFI) for Consulting Services for Patient Centered Medical Home on August 31, 2016. Interested vendors may participate in a Question & Answer conference call on September 14, 2016, 4:00 p.m., ET by dialing 1(877)248-3057, ID: 2070369495. Interested respondents should email dsteinberg@ccpcares.org to confirm attendance of the September 14, 2016 conference call. In order for CCP to respond appropriately during the Question & Answer Session, it is requested that interested respondents make every effort to submit their questions in advance by email to dsteinberg@ccpcares.org no later than 5:00 p.m., ET on September 12, 2016.
The Receipt of Acknowledgment Form must be received by CCP no later than 5:00 p.m., ET, September 16, 2016.
Written follow-up questions must be received by CCP no later than September 21, 2016, 1:00 p.m., ET.
Responses to the RFI must be received by CCP no later than 12:00 Noon, ET, September 30, 2016, by sending proposals to the following:

All responses will be analyzed on the basis of the criteria set forth in the Request for Information.
The evaluation of the proposals received will be conducted from September 30, 2016 to October 7, 2016. Selected vendors will be required to present an oral presentation on October 20, 2016 at CCP. CCP requests that vendors hold this date for on-site presentation.
The expected award date of the RFI is October 2016.
CCP reserves the right to accept or reject, in whole or in part and for any reason whatsoever, any and all responses to the Request for Information and to waive any formalities in the response process.
