SUMMARY: The Department of the Lottery will conduct the Pro Football POWERBALL® Promotion between September 15, 2014, and December 29, 2014, in which special professional football prizes will be awarded.

53ER14-52 Pro Football POWERBALL® Promotion.
(1) Beginning Monday, September 15, 2014, through Monday, December 29, 2014, the Florida Lottery will conduct the Pro Football POWERBALL® Promotion. Lottery players (hereinafter referred to as “player” or “players”) who purchase a single $10.00 or more POWERBALL® or POWERBALL with Power Play® ticket at a Florida Lottery (or “Lottery”) retailer location will receive an entry voucher with a unique number that can be entered into a team drawing in one of three participating professional football team “Experiences” on the Florida Lottery’s website for a chance to win professional football prizes. The professional football team experiences are: the Jacksonville Jaguars Experience, the Miami Dolphins Experience and the Tampa Bay Buccaneers Experience.
(2) Qualifying POWERBALL ticket purchases will produce entry vouchers beginning on September 15, 2014, through 10:40 p.m. ET on December 29, 2014.
(3) Voucher Entry. To enter a voucher number into a team drawing, players must enter on the Lottery’s website at flalottery.com. On the home page of the Lottery’s website, players can click on the Pro Football POWERBALL® Promotion banner and follow the directions. The entry voucher will be attached to the bottom of the qualifying POWERBALL ticket. The entry voucher number is located at the bottom on the front of the entry voucher. Players are to enter the first 13-digits of the 19-digit entry voucher number. A player will be able to enter his or her entry voucher numbers beginning at the top of the hour after the POWERBALL ticket is purchased. A player entering an entry voucher number prior to the top of the hour after purchase will be directed to return at a later time to enter his or her entry voucher number into the drawing. The POWERBALL ticket number cannot be used for entry into the drawing. Players can also access the Lottery’s website to enter a voucher number via a link located on each of the three participating team’s website:

The odds of winning depend on the number of entries received. Entry vouchers should not be mailed to the Lottery unless players are contacted by the Florida Lottery and requested to do so. Entry vouchers or tickets received in the mail by the Florida Lottery will not be entered into the drawing and will not be returned.
(4) Team Experience Drawings. In each of the three team experiences, a total of five drawings will be held between October 1, 2014, and December 31, 2014, from entries submitted before midnight ET on the last day of the entry period. Players may enter as many times as they wish during the contest period. However, each valid voucher entry number may only be used one time, for one entry into one drawing. (a) Drawings 1 through 4. In each drawing, a total of forty valid entries will be drawn. The first and second valid entries drawn will each win a first prize, the third valid entry drawn will win the second prize, the fourth through seventh valid entries drawn will each win a third prize, the eighth through tenth valid entries drawn will each win a fourth prize, and the eleventh through thirtieth valid entries drawn will each win a fifth prize. The thirty-first through fortieth valid entries drawn will serve as alternates in the order in which they were drawn and in the order of need to select an alternate first, second, third or fourth prize winner in the event a first, second, third, or fourth prize cannot be awarded.
(b) Drawing 5. In the fifth drawing, a total of six valid entries will be drawn. The first valid entry drawn will win the suite experience prize. The second through sixth valid entries drawn will serve as alternates in the order in which they were drawn to select an alternate prize winner, in the event the prize cannot be awarded.
(c) All entries are subject to validation by the Florida Lottery and may be disqualified if eligibility requirements are not met.
(5) Winner Notification for Drawings 1 through 4. In each team experience, the prizewinners in the each of the first four drawings will be posted on flalottery.com after the drawings.
(a) First, Second, and Fourth Prize Winners. The Florida Lottery will attempt to notify first, second, and fourth prize winners by telephone, U.S. mail or email no later than twenty-four hours after the winners are posted on the Florida Lottery’s website.
1. For the first three drawings, if the Florida Lottery is unable to have personal contact with a first, second, or fourth prize winner within three days of the date of the drawing, the winner will forfeit his or her right to claim the prize and the prize will be awarded to the first eligible alternate winner in the order drawn. If the Florida Lottery is unable to have personal contact with the alternate winner within three days of the date of award of prize, the alternate winner will forfeit his or her right to claim the prize and the prize will be awarded to the next eligible alternate winner. This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. If the Lottery is unable to contact an alternate, the prize will not be awarded.
2. For the December 3, 2014, drawing, if the Florida Lottery is unable to have personal contact with a first, second or fourth prize winner within two days of the date of the drawing, the winner will forfeit his or her right to claim the prize and the prize will be awarded to the first eligible alternate winner in the order drawn. If the Florida Lottery is unable to have personal contact with the alternate winner within two days of the date of award of prize, the alternate winner will forfeit his or her right to claim the prize and the prize will be awarded to the next eligible alternate winner. This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. For first prize winners, attempts to contact alternates will not be made after December 12, 2014. If the Lottery is unable to contact an alternate, the prize will not be awarded.
(b) Third Prize Winners. The Florida Lottery will attempt to notify a third prize winner by telephone, U.S. mail or email no later than twenty-four hours after the winners are posted on the Florida Lottery’s website. If the Florida Lottery is unable to have personal contact with the third prize winner within seven days of the date of the drawing, the winner will forfeit his or her right to claim the prize and the prize will be awarded to the first eligible alternate winner in the order drawn. If the Florida Lottery is unable to have personal contact with the alternate winner within seven days of notification, the alternate winner will forfeit his or her right to claim the prize and the prize will be awarded to the next eligible alternate winner. This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. If the Lottery is unable to contact an alternate the prize will not be awarded.
(c) Fifth Prize Winners. The Florida Lottery will attempt to notify each fifth prize winner by telephone, U.S. mail, or email no later than twenty-four hours after the winners are posted on the Florida Lottery’s website in order to confirm the winner’s shipping address. If the Florida Lottery is unable to have personal contact with a fifth prize winner within sixty days of the date of the drawing, the winner will forfeit his or her right to claim the prize and the prize will not be awarded.
(6) Winner Notification for Drawing 5. In each team experience, the suite experience prize winner in the fifth drawing will be posted on flalottery.com after the drawing. The Florida Lottery will attempt to notify the suite experience prize winner by telephone, U.S. mail or email no later than twenty-four hours after the winners are posted on the Florida Lottery’s website. If the Florida Lottery is unable to have personal contact with the suite experience prize winner within seven days of the date of the drawing, the winner will forfeit his or her right to claim the prize and the prize will be awarded to the first eligible alternate winner in the order drawn. If the Florida Lottery is unable to have personal contact with the alternate winner within seven days of notification, the alternate winner will forfeit his or her right to claim the prize and the prize will be awarded to the next eligible alternate winner. This process will continue until an alternate is contacted or the Florida Lottery has exhausted the list of available alternates. If the Lottery is unable to contact an alternate the prize will not be awarded.
(7) Notification of Alternates. The Florida Lottery will notify each alternate in the team experience drawings by email no later than twenty-four hours after the drawing. The email will notify the alternate of his or her alternate status and instruct the alternate to retain the original valid entry voucher that is required to claim a prize, until further notified by the Florida Lottery that he or she is an alternate winner of a prize or that the prizes have been fulfilled and the original valid entry voucher may be discarded.
(8) How to Claim a Prize.
(a) To claim a first, second, or fourth prize in one of the first three team drawings, a winner must submit for receipt by the Florida Lottery no later than five days after being notified by the Lottery that he/she is a winner, the original valid voucher bearing the unique number selected in the drawing, a completed Winner Claim Form DOL-173-2, revised 09/13, or DOL-173-2S, revised 09/13, and a copy of acceptable identification as set forth in the rule of the Florida Lottery governing payment of prizes. A copy of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tallahassee, Florida 32399-4011. Forms DOL-173-2 and DOL-173-2S are hereby incorporated by reference and can be obtained from any Lottery office, from the Lottery’s website, flalottery.com., or by writing to: Florida Lottery, Customer Service Division, 250 Marriott Drive, Tallahassee, Florida 32399-4016. First and second prize winners must also submit a completed Release and Authorization form DOL-474, revised 8/13, or Spanish Release and Authorization form DOL-474S, revised 8/13. Forms DOL-474 and DOL-474S are hereby incorporated by reference and can be obtained from any Lottery office or from the Lottery’s website, flalottery.com.
(b) To claim a first or fourth prize in the December 3, 2014, drawing, a winner must submit for receipt by the Florida Lottery no later than three days after being notified by the Lottery that he/she is a winner, the original valid voucher bearing the unique number selected in the drawing and the required documentation for a first or fourth prize as set forth in paragraph (8)(a) above.
(c) To claim the second prize in the December 3, 2014, drawing, the winner must submit for receipt by the Florida Lottery no later than four days after being notified by the Lottery that he/she is a winner, the original valid voucher bearing the unique number selected in the drawing and the required documentation for a second prize as set forth in paragraph (8)(a) above.
(d) To claim a third prize in any of the first four team drawings or the suite experience prize in the fifth drawing, the winner must submit for receipt by the Florida Lottery no later than fourteen days after being notified by the Lottery that he/she is a winner, the original valid voucher bearing the unique number selected in the drawing, a completed Winner Claim Form and a copy of acceptable identification.
(e) Fifth prize winners are not required to submit the original valid voucher bearing the unique number selected in the drawing or any documentation listed in (8)(a) in order to claim a prize.
(f) A prize winner who cannot produce a valid entry voucher and/or who does not submit the required documents for receipt by the Lottery within the applicable timeframes set forth above will forfeit his or her right to claim the prize and the prize will be awarded to an alternate winner as described in subsections (4), (5) and (6), as applicable.
(9) Jacksonville Jaguars (“Jaguars”) Prizes.
(a) Prizes in Drawings 1 through 4.
1. First Prize: “$10,000 Cash Giveaway Finalist.” First prize consists of two lower level tickets to the December 18, 2014, Jacksonville Jaguars vs. Tennessee Titans home game designated as the “$10,000 Cash Giveaway” game. At the game, the winner will be a finalist in the on-field giveaway and will win up to $10,000 in cash. First prize also includes two Jaguars hats and two Jaguars shirts. (Total value $350 plus cash won in the $10,000 Cash Giveaway)
2. Second Prize: “Flyaway Trip.” Second prize consists of roundtrip airfare for two from any U.S. regional or international airport with regularly scheduled commercial service to the city in which the away game will be held*, two tickets to the December 28, 2014, Jacksonville Jaguars vs. Houston Texans away game**, one standard hotel room for two persons for two nights, transportation to and from the airport and to and from the game, and a $500 merchandise credit for use at the in-stadium Jaguars store. (Estimated total value $3,000***)
3. Third Prize: “2015 Season Tickets.” Third prize consists of two Jaguars lower level season tickets for the 2015 season. (Total value $1,458)
4. Fourth Prize: “2014 VIP Game Experience.” Fourth prize consists of two Jaguars lower level 2014 single game tickets, two pre-game hospitality credentials and one parking pass. For the first three drawings, the VIP game experience will be for the December 7, 2014, Jacksonville Jaguars vs. Houston Texans home game. For the fourth drawing, the VIP game experience will be for the December 18, 2014, Jacksonville Jaguars vs. Tennessee Titans home game****. (Total value $465)
5. Fifth Prize: “Scratch Tickets and Merchandise.” Fifth prize consists of $100 in Lottery Scratch-Off tickets, two Jaguars hats and two Jaguars shirts. (Total value $250)
*No monetary compensation will be given to the winner in lieu of airfare if the away game is held in a city for which air travel from the winner’s city of residence is not required. In such case, the value of the prize for tax reporting will be reduced by the estimated value of the airfare.
** If the second prize in the December 3, 2014, drawing is not claimed in time for the winner to use the trip for the December 28, 2014, away game, the trip will be to an away game (to be determined by the Jaguars) during the 2015 season.
***The actual value of the prize depends on the cost of airfare based on the location of the winner and the location of the game.
****If a fourth prize in the December 3, 2014, drawing is not claimed in time for the winner to use the VIP game experience for the December 18, 2014, home game, the VIP game experience will be for a home game (to be determined by the Jaguars) during the 2015 season.
(b) Prize in Drawing 5. One VIP Suite Experience Prize will be awarded in Drawing 5. A suite experience prize consists of single game day tickets for eight people in a catered suite. A suite experience prize does not include alcoholic beverages. The specific 2015 Jaguars home game will be determined by the Jaguars. Persons under the age of eighteen must be accompanied by an adult. (Total value $5,000)
(10) Miami Dolphins (“Dolphins”) Prizes.
(a) Prizes in Drawings 1 through 4.
1. First Prize: “$10,000 Cash Giveaway Finalist.” First prize consists of two lower level tickets to the December 21, 2014, Miami Dolphins vs. Minnesota Vikings home game designated as the “$10,000 Cash Giveaway” game. At this game, the winner will be a finalist in the on-field giveaway and will win up to $10,000 in cash. First prize also includes two Dolphins hats and two Dolphins shirts. (Total value $400 plus cash won in the $10,000 Cash Giveaway)
2. Second Prize: “Flyaway Trip.” Second prize consists of roundtrip airfare for two from any U.S. regional or international airport with regularly scheduled commercial service to the city in which the away game will be held*, two tickets to the December 14, 2014, Miami Dolphins vs. New England Patriots away game**, one standard hotel room for two persons for two nights, $200 gift card for transportation, and a $500 merchandise credit for use at the online or in-stadium Dolphins store. (Estimated total value $4,000***)
3. Third Prize: “2015 Season Tickets.” Third prize consists of two Dolphins lower level season tickets for the 2015 season. (Total value $1,640)
4. Fourth Prize: “2014 VIP Game Experience.” Fourth prize consists of two Dolphins lower level 2014 single game tickets, two pre-game hospitality credentials and one parking pass. The VIP game experience will be for the December 28, 2014, Miami Dolphins vs. New York Jets home game****. (Total value $1,000)
5. Fifth Prize: “Scratch Tickets and Merchandise.” A fifth prize consists of $100 in Lottery Scratch-Off tickets, two Dolphins hats and two Dolphins shirts. (Total value $200)

(11) Tampa Bay Buccaneers (“Buccaneers”) Prizes.
(a) Prizes in Drawings 1 through 4.
1. First Prize: “$10,000 Cash Giveaway Finalist.” First prize consists of two lower level tickets to the December 21, 2014, Tampa Bay Buccaneers vs. Green Bay Packers home game designated as the “$10,000 Cash Giveaway” game. At the game, the winner will be a finalist in the on-field giveaway and will win up to $10,000 in cash. First prize also includes two Buccaneers hats and two Buccaneers shirts. (Total value $198 plus cash won in the $10,000 Cash Giveaway)
2. Second Prize: “Flyaway Trip.” Second prize consists of a roundtrip chartered flight for two with the Buccaneers, two tickets to the December 14, 2014, Tampa Bay Buccaneers vs. Carolina Panthers away game*, roundtrip shuttle transportation to and from the game and a $500 merchandise credit for use at the online or in-stadium Buccaneers store. (Total value $2,500)
3. Third Prize: “2015 Season Tickets.” Third prize consists of two Buccaneers lower level season tickets for the 2015 season. (Total value $1,500)
4. Fourth Prize: “2014 VIP Game Experience.” Fourth prize consists of two Buccaneers lower level 2014 single game tickets, two pre-game tailgate passes and one parking pass. The VIP game experience will be for the December 28, 2014, Tampa Bay Buccaneers vs. New Orleans Saints home game**. (Total value $235)
5. Fifth Prize: “Scratch Tickets and Merchandise.” Fifth prize consists of $100 in Lottery Scratch-Off tickets, two Buccaneers hats and two Buccaneers shirts. (Total value $148)
* If the second prize in the December 3, 2014, drawing is not claimed in time for the winner to use the trip for the December 14, 2014, away game, the trip will be to an away game (to be determined by the Buccaneers) during the 2015 season.
**If a fourth prize in the December 3, 2014, drawing is not claimed in time for the winner to use the VIP game experience for the December 28, 2014, home game, the VIP game experience will be for a home game (to be determined by the Buccaneers) during the 2015 season.
(b) Prize in Drawing 5. One VIP Suite Experience prize will be awarded in Drawing 5. A suite experience prize consists of single game day tickets for eight people in a catered suite. A suite experience prize does not include alcoholic beverages. The specific 2015 or 2016 Buccaneers home game will be determined by the Buccaneers. (Total value $10,500)
(12) Award of Team Prizes,
(a) First Prize. Upon the Florida Lottery’s receipt of a first prize winner’s required documentation, the Lottery will provide the winner a certificate with the necessary information to obtain the team football tickets from the fulfillment entity and provide the winner details of the $10,000 Cash Giveaway. The Florida Lottery will ship the winner the team hats and shirts. In the $10,000 Cash Giveaway, eight finalists attending the 2014 designated team home game will participate in an in-stadium half-time event in which the winner of $10,000 in cash will be determined. Each of the eight finalists will pull off a tab on an oversized symbol to reveal his or her cash prize. The prizes and number of winners in the $10,000 Cash Giveaway are as follows:

For the $10,000 prize winner in the $10,000 Cash Giveaway, the Florida Lottery will withhold applicable federal tax withholding on the total value of the first prize ($10,000 cash plus the value of the game tickets and the team merchandise). For the remaining cash prize winners in the $10,000 Cash Giveaway, all federal, state and/or local taxes or other fees on cash prizes won plus the value of the game tickets and the team merchandise will be the responsibility of the winner. If a first prize winner fails to arrive at the game for his or her scheduled participation in the $10,000 Cash Giveaway, the first prize winner shall forfeit the right to personally participate in the $10,000 Cash Giveaway; however, a proxy appointed by the Lottery shall participate in the first prize winner’s place and the cash prize won by the proxy shall be awarded and taxable to the first prize winner.
(b) Second Prize. Upon the Florida Lottery’s receipt of a second prize winner’s required documentation, the Lottery will provide the winner a certificate describing the team Flyaway Trip package along with the necessary information to make reservations to fulfill the trip and information on use of the $500 team merchandise credit. As applicable to each team’s second prize specifics, a second prize does not include: mileage, insurance, gratuities, meals (other than those provided without charge), parking fees, baggage fees, alcoholic beverages, any items not expressly specified and personal expenses such as telephone calls, valet service, laundry, incidentals and the like, as well as revision or cancellation fees which may be charged by the airlines, hotel or other supplier.
(c) Third Prize. Upon the Florida Lottery’s receipt of a third prize winner’s required documentation, the Lottery will provide the winner a certificate with the necessary information to obtain the team tickets from the fulfillment entity.
(d) Fourth Prize. Upon the Florida Lottery’s receipt of a fourth prize winner’s required documentation, the Lottery will provide the winner a certificate with the necessary information to obtain the team tickets, pre-game hospitality passes or credentials and parking pass from the fulfillment entity. Pre-game hospitality does not include alcoholic beverages.
(e) Fifth Prize. Upon confirmation of the winner’s mailing address, the Florida Lottery will ship the winner $100 in Lottery Scratch-Off tickets and the team hats and shirts. A winner whose mailing address is outside the state of Florida will receive a check for the value of the Lottery Scratch-Off tickets portion of the prize in lieu of the actual tickets.
(f) Suite Experience Prize. Upon the Florida Lottery’s receipt of a suite experience prize winner’s required documentation, the Lottery will provide the winner a certificate with the necessary information to obtain the team suite game day football tickets (and parking passes for the Dolphins suite experience prize winner) from the fulfillment entity.
(13) Taxes. Except as specifically mentioned herein, all federal, state and/or local taxes or other fees on the prizes will be the responsibility of the winner. Federal income taxes are required to be withheld from a prize awarded to a nonresident alien claimant at the rate of thirty percent (30%) pursuant to applicable provisions of the Internal Revenue Code. A nonresident alien claimant who is selected as a winner of a prize for which tax withholding is not paid by the Florida Lottery will be required to pay the withholding tax or forfeit the prize. The reporting and subsequent payment of any additional federal, state and/or local taxes shall be the responsibility of the nonresident alien winner.
(14) State Owed Debt. A first prize winner will be analyzed for State Owed Debt after the $10,000 Cash Giveaway event has concluded and each winning amount has been revealed. If the winner is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with Section 24.115, F.S. If the debt is an amount less than the cash prize, the amount owed and federal tax withholding, if applicable, shall be deducted and the remaining cash prize will be awarded. If the winner is identified as owing such a debt in an amount greater than the prize, the winner’s entire cash prize will be applied toward the outstanding debt as provided in Section 24.115, F.S.
(15) Other Restrictions and Provisions.
(a) No cash option is available in lieu of the non-cash prizes.
(b) The right to claim a prize cannot be assigned to another person or entity.
(c) All prizes are subject to the provisions of Chapter 24, Florida Statutes, and rules promulgated thereunder. Prizes will be paid in accordance with the rule of the Florida Lottery governing payment of prizes. Copies of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tallahassee, Florida 32399-4011.
(d) Players must be at least 18 years of age. Persons prohibited by Section 24.116, F.S., from purchasing a Florida Lottery ticket are not eligible to play.
(e) By entering the Pro Football POWERBALL® Promotion, a player gives his or her permission for the Florida Lottery to provide the player’s address and telephone number to the fulfillment entity for prize fulfillment purposes.
(f) A player entering the Pro Football POWERBALL® Promotion is deemed to have granted permission for the Florida Lottery to photograph and/or videotape and record the prizewinner with or without prior notification and to use the name, photograph, videotape, and/or recording of the prizewinner for advertising or publicity purposes without additional compensation.
(g) The Pro Football POWERBALL® Promotion drawings shall be public, held in Tallahassee, Florida, and witnessed by an accountant employed by an independent certified public accounting firm.
