NOTICE IS HEREBY GIVEN that on September 2, 2014, the Department of Children and Families received a petition for waiver of rule 65D-30.003, 65D-30.004 and 65D-30.007, Florida Administrative Code, from Holistix by the Sea, LLC. Rules 65D-30.003, 65D-30.004 and 65D-30.007 pertain to Residential Level 5 programs that provide only housing utilized solely for the purpose of assisting clients in making a transition to independent living while attending the least intense levels of outpatient treatment.
