PURPOSE AND EFFECT: The Board proposes the development of rule amendments to address the revised financial responsibility form to make the form consistent with the statute. The form is also being updated in the various applications which require the demonstration of financial responsibility. The Board also proposes updating several of the applications to delete the military information (form DH-MQA 1009) and to include additional questions with regard to the 5th pathway program and delete the requirement for undergraduate school transcripts (form DH-MQA 1000).
