Invitation for Rate Agreement Proposals for Adult Day Care Service

Senior Friendship Centers is requesting Rate Agreement Proposals from agencies located in Lee County; licensed with the Agency for Health Care Administration to provide Adult Day Care Services for those 60 and older and/or diagnosed/suspected of having Alzheimer’s or a related disorder.
These services are to be funded by the Florida Department of Elder Affairs and applicants must adhere to all state requirements. The contract period will be effective October 15, 2014 through June 30, 2018 with (3) one year renewals. All interested agencies may submit a Notice of Intent to obtain a Rate Agreement package by emailing cw@friendshipcenters.org, at Senior Friendship Centers, by 12:00 Noon, September 5, 2014. Packages (with all rate agreement instructions) will be emailed to those interested on Monday, September 8, 2014.

All agreement proposals must be received by 3:00 p.m. on Monday, September 29, 2014 in a sealed envelope addressed as: “ADC Rate Agreement Proposal”, Senior Friendship Centers, Inc. Attention: Nancy Green-Irwin, 5272 Summerlin Commons Way, #604, Ft. Myers, FL 33907.
Proposals will be opened publicly at 10:00 a.m. on Tuesday, September 30, 2014 at 5272 Summerlin Commons Way, #604, Ft. Myers, FL 33907.
