    64B18-11.001Application for Licensure

    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 42 No. 123, June 24, 2016 issue of the Florida Administrative Register.

    The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee and by a discussion and vote by the Board at its September 9, 2016 meeting. The rule shall now read as follows:

    64B18-11.001 Application for Licensure.

    (1) Any person desiring to be licensed as a podiatric physician shall apply to the Board of Podiatric Medicine. The application shall be made on the Application for Podiatric Examination & Initial Licensure Form DH-MQA 1138 (revised 07/2016 05/2016), hereby adopted and incorporated by reference, that can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref- and the web at http://www.doh.state.fl.us/mqa/podiatry/index.html.

    (2) No change.

    The following changes have been made to incorporated form DH-MQA 1138, 05/2016:

    • The revision date of the form has been changed to 07/2016.
    • The coding of changes has been removed from the form.
    • Page 6, number 3.d.: the request for the e-mail address has been changed to optional. Language has been added that providing this information is a public record.
    • Page 11, number 22: the word “felon” has been changed to “felony.”
