NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 46 No. 91, May 8, 2020 issue of the Florida Administrative Register.
The Board received written comments from the Joint Administrative Procedures Committee on Rule 64B6-8.002. Additionally, the Board made changes to the proposed form set forth in the proposed rule.
The changes to the form are as follows:
64B6-8.002(1)(b):Form DH-MQA 1158 cites chapter 64B6-8 rather than “Rule 64B6-8.” Form DH-MQA 1158 has removed requirements of affirmations by the Applicant.
