NOTICE IS HEREBY GIVEN that on September 9, 2016, the Board of Optometry, received a petition for variance or waiver filed by William A. Monaco requesting a permanent waiver by endorsement of Rule 64B13-4.001, F.A.C., regarding the requirement that applicants for licensure must have achieved all three portions of Part III on the same test attempt score of 75% or better on all 4 parts of the licensure examination within the seven (7) year period immediately preceding application for licensure.
