NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 41 No. 137, July 16, 2015 issue of the Florida Administrative Register.

No change to the rule text.

 

The following changes were made to the Application for Florida Birth Record – County Use, DH 1960, incorporated by reference: All references to tax collector offices have been omitted and the description of possible surcharges levied by tax collectors have been omitted. The section entitled “Unique County Information” on page 1 has been changed to “County Health Department Fee Information.” “Unique County Information” on page 2 has been changed to “County Health Department Name and Address.”

 

The following changes were made to the Application for Florida Death Record – DH 1961, incorporated by reference: The section entitled “Unique County Information” on page 1 has been changed to “County Health Department Fee Information.” “Unique County Information” on page 2 has been changed to “County Health Department Name and Address.”
