PUBLIC ANNOUNCEMENT FOR CONSTRUCTION MANAGEMENT SERVICES

PROJECT NAME:Jacksonville Laboratory Porter Building Renovations
PROJECT LOCATION:1217 Pearl Street, Jacksonville, FL 32202
The State of Florida, Department of Health, Division of Administration, Bureau of General Services, Office of Design and Construction requests qualifications from construction management firms to provide construction management services for this project. The estimated construction budget for this project is Four Million, Five Hundred Thousand Dollars ($4,500,000).
Work Area: First and second floors of an existing three-story building, approximately 9000 square feet per floor.
General Description of Base Scope of Work: Demolish the first and second floor down to the concrete structural frame. Rebuild all interior architectural features; install new mechanical, electrical, and plumbing systems. Renovate the exterior brickwork and install new window systems (refer to attached general information for further details.
Additional Work: Dependent on available funds and at the sole discretion of the Department, the project scope of work may be expanded to include renovations to the campus auditorium and other projects in the laboratory campus. The estimated construction budget for the additional work is One Million, Five Hundred Thousand Dollars ($1,500,000). If funding becomes available and the selected firm is assigned the additional work, the various components will be negotiated as separate GMP deliverables.
The anticipated work period for the base work is Eighteen (18) months. The time may be shortened or lengthened in the best interest of the Department.
The anticipated construction start date is April 2021. Applicant must be a licensed general contractor in the State of Florida at the time of application. Further, if a corporation, the applicant must be registered by the Department of State, Division of Corporations, to operate in the State of Florida at the time of application.
APPLICATION INSTRUCTIONS:
The application shall be a maximum of 50 pages (excluding the table of contents, blank pages, section tab sheets and covers). All pages after 50 shall be marked NOT APPLICABLE and will not be a considered in the review process.
Firms interested in being considered for this project must submit Five (5) printed copies of their application with a table of contents and tabbed sections in the following order:
1. Letter of interest with an overview of the firm’s qualifications to meet the above referenced selection criteria.
2. A completed Experience Questionnaire and Financial Information Form (DOH-EQFI). A copy may be obtained by calling Kathryn Vowell at (850)245-4066.
3. Résumés of the proposed key personnel and staff. A standard resume form (DOH-330R) may be obtained by calling Kathryn Vowell at (850)245-4066.
4. Staff organization charts (key personnel) for both the preconstruction service phase and the construction service phase. Graphic presentation is recommended.
5. A statement (text or graphic) showing map mileage (use FDOT mileage chart) from your proposed office to the job site.
6. Examples of the firm’s work experience with a focus on completed projects similar
rto the Porter Building project. Photos and graphic explanations are encouraged.7. Examples of the firm’s ability to document work progress and project status. Describe how your firm’s management systems integrate with the DMS-Project Management Information System (PMIS). Graphic examples are encouraged.
8. References from prior clients received within the last five years.

SHORTLIST SELECTION PROCESS: From the proposals received, the Department shall shortlist a minimum of three (3) firms.
All proposal information submitted becomes the property of the Department of Health and will be placed on file. The submitted documents will become available for public review once the selection process has completed. Applications which do not comply with the instructions set forth above and/or do not include the qualification data required will be considered improper and disqualified. Selections will be made in accordance with Section 287.055, Florida Statutes.
The qualified, selected firm will be required to be registered with MyFloridaMarketPlace before a contract can be executed. However, they will be exempt from the one per cent fee.
