REQUEST FOR PROPOSAL
South Florida Community Care Network, LLC d/b/a Community Care Plan (CCP) is issuing a Request for Proposal (RFP) for Transportation Services for Medicaid Members on September 7, 2018. Interested vendors may participate in a Question & Answer conference call on September 18, 2018, 1:00 p.m. (ET) by dialing (240)454-0879, ID/Access Code: 734400839. Interested vendors should email: jmcdaniel-chinn@ccpcares.org to confirm attendance of the September 18, 2018 conference call. In order for CCP to respond appropriately during the Question & Answer session, it is requested that interested vendors make every effort to submit their questions in advance by email to jmcdaniel-chinn@ccpcares.org no later than 5:00 p.m. (ET) on September 14, 2018.
The Receipt of Vendor Acknowledgment Form must be received by CCP no later than 5:00 p.m. (ET) on September 20, 2018.
Written follow-up questions must be received by CCP no later than 1:00 p.m. (ET) on September 23, 2018.
Responses to the RFP must be received by CCP no later than 1:00 p.m. (ET) on September 28, 2018, by sending proposals to the following:

All responses will be analyzed on the basis of the criteria set forth in the Request for Proposal.
The evaluation of the proposals received will be conducted from September 28, 2018 to October 5, 2018. Selected vendors will be required to present an oral presentation on October 9, 2018 at CCP. CCP requests that respondents hold this date for an on-site presentation.
The expected award date of the RFP is November 2018.
CCP reserves the right to accept or reject, in whole or in part and for any reason whatsoever, any and all responses to the Request for Proposal and to waive any formalities in the response process.
