NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 41 No. 147, July 30, 2015 issue of the Florida Administrative Register.

This change is a revision to the proposed rule text in response to concerns presented by the Joint Administrative Procedures Committee.

 

64B-9.006 Massage Establishment Licensure; Ownership Information.

For purposes of complying with Section 480.043(2), F.S., a massage establishment must submit the following documentation as proof of having more than $250,000 of business assets in this state:

(1) A formal opinion letter from a Florida licensed certified public accountant confirming affirming the corporation had more than $250,000 of business assets during the previous tax year; or

(2) A copy of the corporation’s Florida Corporate income/Franchise Tax Return (form F-1120) from the previous tax year.
