64-3.015Special Needs Shelter Guidelines
NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 42 No. 222, November 15, 2016 issue of the Florida Administrative Register.
Said changes are in response to written comments submitted by the Joint Administrative Procedures Committee staff:
64-3.015 Special Needs Shelter Guidelines.
(1) The Department hereby adopts the publication “Guidelines for Special Needs Shelters” (December 1, 2016 edition), incorporated by reference and available at http://www.flrules.org/Gateway/reference.asp?No=Ref-#####. Except for this rule section, this publication contains the Department guidelines rules in this chapter applicable to Special Needs Shelters.
(2) Health care practitioners and other vendors shall request reimbursement for services rendered by submitting Form DH 1989 (January 2017 edition), “Vendor Invoice for Special Needs Health Care Practitioner,” or Form DH 1990 (January 2017 edition), “Vendor Invoice for Services Rendered to Special Needs Clients Placed by the Multiagency Special Needs Shelter Discharge Planning Team,” as applicable. These forms are incorporated by reference and are available at http://www.flrules.org/Gateway/reference.asp?No=Ref-##### and http://www.flrules.org/Gateway/reference.asp?No=Ref-#####.
The following legal citation and history note line for this rule section should read as follows:

The publication referenced above entitled “Guidelines for Special Needs Shelters” (December 2016 edition) (“Guidelines”) is hereby changed in the following manner:
The Guidelines section titled “Special Needs Shelter Registration,” Sub-unit (5), is changed to delete a reference to Rule 64‑3.020(1), F.A.C.
The Guidelines section titled “Service Reimbursement” is changed to remove language that would appear to incorporate Forms DH 1989 (12/06) and DH 1990 (12/06) by reference. The change provides a parenthetical reference to the rule text section (64-3.015) where these forms are updated and incorporated. Additionally, the logo is updated on the two forms as well.
This same Guidelines section is corrected to update the citations to current subsections of Section 381.0303, Florida Statutes, in the Guidelines text.
