Summary:
The proposed rule amendments clarify the appropriate content for medical records by requiring sufficient detail to demonstrate the course of treatment by the physician; requiring copies of documents from other health care practitioners relied upon for treatment of the patient; and setting forth requirements for late entries into the patient’s medical record.
