    64B16-26.204Licensure by Endorsement

    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 42 No. 141, July 21, 2016 issue of the Florida Administrative Register.

    The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee and discussion and subsequent vote by the board at the public meeting held October 18, 2016. The rule shall now read as follows:

    64B16-26.204 Pharmacist Licensure by Endorsement; Application.

    (1)(a) U.S. Graduates. All applications for licensure by endorsement for pharmacists who received a degree from a school or college of pharmacy accredited by an accrediting agency recognized and approved by the United States Department of Education shall be made on board approved form DOH-MQA 100, 10/16 07/16,  Pharmacist Licensure by Endorsement Application and Instructions (U.S. Graduates), which is hereby incorporated by reference, and which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-   ,  the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board's website at http://floridaspharmacy.gov/Applications/app-pharmacist-endorse-uspr-grad.pdf.  

    (b) Foreign Graduates. All applications for licensure by endorsement for pharmacists graduated from a pharmacy program of a school or college of pharmacy located outside of the United States shall be made on board approved form DOH-MQA 1196, 10/16 07/16,  Pharmacist Licensure by Endorsement Application and Instructions (Foreign Graduates), which is hereby incorporated by reference, and which can be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-      ,   the Board of Pharmacy, 4052 Bald Cypress Way, Bin #C04, Tallahassee, Florida 32399-3254, or the Board's website at http://floridaspharmacy.gov/Applications/app-pharmacist-endorse-foreign-grad-2013.pdf, 

    (2) The application must be accompanied with a non-refundable application fee and initial licensure fee as set forth in Rules 64B16-26.1001 and 64B16-26.1002, F.A.C., and in addition, the applicant must submit satisfactory proof of the following:

    (a) Compliance with the requirements of subsections 465.007(1)(b)1. or 2. and (1)(c), F.S.;

    (b) Having obtained a passing score on a licensure examination as described in subsection 465.0075(1)(b), F.S.; and

    (c) 1. Having two (2) years of active practice, as defined in Section 465.0075(1)(c)1., F.S., within the immediately preceding five (5) years and have completed 30 hours of Board approved continuing education within in the two (2) calendar years immediately preceding application. For purposes of this rule only, the Board defines “Board approved continuing education” as any continuing education course offered, sponsored, or approved by the Florida Board of Pharmacy, the Accreditation Council for Pharmacy Education (ACPE), or other state board of pharmacy which course was accepted by the other state board for licensure or licensure renewal purposes; approved continuing education within in the two (2) calendar years immediately preceding application;

    2. No change. 

    3. Successful completion of an internship meeting the requirements of Section 465.007(1)(c), F.S., and Rule 64B16-26.2033, F.A.C. within the two (2) years immediately preceding application.

    (3) through (5) No change.

    The following changes have been made to incorporated form DOH-MQA 100:

    The effective date of the form has been changed to 10/16.

    The form name has been changed to DOH-MQA 100.

    The pages of the form have been renumbered sequentially.

    On page 1 of 17, under General Information, number 1 c: the words “as required above” have been removed.

    On page 1 of 17, under General Information, number 2, the name of the examination has been corrected to North American Pharmacist Licensure Examination.

    On pages 4, 6, and 8 of 17, the words “Social Security Form” have been changed to “Social Security Number.”

    On page 10 of 17, Number 9, a reference to section 456.38 has been added.

    On page 14 of 17, the citation to section 456.015(2)(a) has been corrected to 465.015(2)(a).

     

    The following changes have been made to incorporated form DOH-MQA 1196:

    The effective date of the form has been changed to 10/16.

    The form name has been changed to DOH-MQA 1196.

    The pages of the form have been renumbered sequentially.

    On page 2 of 18, number 3, the name of the examination has been corrected to North American Pharmacist Licensure Examination.

    On page 7 of 18, the words “Social Security Form” have been changed to “Social Security Number.”

    On page 7 of 18, the request for a copy of the Social Security card has been removed.

    On page 11 of 18, a reference to section 456.38, F.S., has been added.

    On page 14 of 17, the citation to section 456.015(2)(a) has been corrected to 465.015(2)(a).
