    SUMMARY: The proposed emergency rule incorporates the mandatory informed consent form which must be utilized by qualified physicians who certify patients for medical marijuana.

    64B8ER17-1 Mandatory Standardized Informed Consent for Medical Marijuana.  Pursuant to Section 381.986, F.S., the Board has approved form DOH-MQA-5026 (8/17), entitled “Medical Marijuana Consent Form,” which is hereby incorporated by reference and available from the Board’s website at http://www.flboardofmedicine.gov/forms/medical-marijuana-consent-form.pdf, as the mandatory standardized informed consent form that a qualified physician must use each time the qualified physician issues a certification for medical marijuana to a patient he or she has diagnosed with at least one qualifying medical condition.

    THIS RULE TAKES EFFECT UPON BEING FILED WITH THE DEPARTMENT OF STATE UNLESS A LATER TIME AND DATE IS SPECIFIED IN THE RULE.
