NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 41 No. 230, November 30, 2015 issue of the Florida Administrative Register. Incorporated Form DH1276-MQA (effective 9/2015) has been revised to correct the effective date, to correct the title and to remove a time period limitation on questions relating to criminal history and Medicare/Medicaid fraud. The following changes are made to the rule text.
64B23-5.002 Procedure for Reactivation of Inactive Status to Active Status.
(1) A licensee may apply to reactivate an inactive license on Form #DH 1276-MQA, Reactivation Application for Reactivation (9/2015) , 7/15, incorporated by reference and available at http://flrules/Gateway/reference.asp?No-Ref-#####.
(2) through (3) No change.
