    53ER16-21 WHEEL OF FORTUNE® BIG MONEY Second Chance Promotion.

    (1) Second Chance Promotion. Beginning April 5, 2016, and continuing through June 28, 2016, the Florida Lottery will conduct the WHEEL OF FORTUNE® BIG MONEY Second Chance Promotion (“Promotion”) in which players can enter their non-winning WHEEL OF FORTUNE  Florida Lottery Scratch-Off lottery tickets (Game 5010) on the Florida Lottery website for a chance to win one of twelve Grand Prize VIP Trip Packages to Los Angeles, California, and a chance to be selected as a contestant in a lottery players-only, non-broadcast WHEEL OF FORTUNE game show and, if selected,  play to win up to $1,000,000. Winning WHEEL OF FORTUNE tickets cannot be used for entry in the Promotion.

    (2) How to Enter.

    (a) To enter a non-winning WHEEL OF FORTUNE Scratch-Off ticket into one of the four second chance drawings further described in subsection (3) below, players must enter on the Florida Lottery’s website at flalottery.com. On the home page of the Florida Lottery’s website, players can click on the WHEEL OF FORTUNE banner and follow the directions. Players will be prompted to log in or register. The 24-digit ticket serial number (4-digit game number and 20-digit ticket number) is located at the bottom on the front of a WHEEL OF FORTUNE Scratch-Off ticket under the latex covering. Players are to scratch off the latex covering to reveal the number and enter the entire 24-digit ticket number in the designated ticket entry area on the website.

    (b) Players may enter as many times as they wish during the contest period; however, each valid ticket number may only be used one time, for one opportunity to earn one entry into one second chance drawing and a chance to win bonus entries as further described in paragraph (2)(c) below.  Tickets should not be mailed to the Lottery unless players are contacted by the Florida Lottery and requested to do so. Non-winning tickets received in the mail by the Florida Lottery will not be entered in the drawing and will not be returned. The odds of winning depend on the number of entries entered. All entries are subject to validation by the Florida Lottery and may be disqualified if eligibility requirements are not met.

    (c) “Buy a Vowel” Option. At the time of entering a non-winning WHEEL OF FORTUNE ticket, players will be provided an option to play the “Buy a Vowel” play-for-fun interactive game (“Buy a Vowel Game”) on the website for a chance to win bonus entries. For each non-winning ticket entered, the player will receive one vowel to be used towards completion of a WHEEL OF FORTUNE puzzle board. If the vowel can be used on the puzzle board, it will automatically populate the puzzle board; otherwise, the vowel will be banked for future use. Banked vowels will carry over from one Buy a Vowel Game to the next and from one non-winning ticket entry to the next.  Once a puzzle board is completed, players can spin a bonus wheel to receive from 2 to 50 bonus entries which will automatically be entered into the next available second chance drawing. If players choose not to play the Buy a Vowel game or if they experience technical problems while playing the game, any bonus entries the players may have won in the course of playing the Buy a Vowel Game will be credited to their entry account.

    (3) Drawings and Prizes. A total of four second chance drawings will be held and winners will be randomly selected from entries submitted before midnight ET on the last day of the entry period for each drawing. The entry periods and drawing dates are set forth in the table below.

    A total of 12 prizes will be available in the Promotion.

    In each of the four drawings, a total of ten valid entries will be drawn by the Florida Lottery using a certified random number generation process. The first three valid entries drawn will each win a Grand Prize VIP Trip Package. The fourth through the tenth valid entries drawn will be used in the order in which they were drawn to select an alternate grand prize winner in the event a grand prize cannot be awarded.

    (4) Notification.

    (a) The names of the grand prize winners in each second chance drawing will be posted on flalottery.com after the drawing.

    (b) The Florida Lottery will attempt to notify each grand prize winner by telephone or email using the contact information provided in the winner’s registration data no later than twenty-four hours after the winners are posted on the Florida Lottery’s website. The Florida Lottery deems the winner’s registration data as the winner’s official contact information, including the physical mailing address, and will not attempt to further locate a winner if attempts to reach the winner using the winner’s registration data are unsuccessful. If the Florida Lottery is unable to have personal contact with the grand prize winner within two weeks of the date of the drawing, the winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to the first alternate winner in accordance with subsection (3) above.  If the Florida Lottery is unable to have personal contact with the first alternate winner within two weeks of the date of notification, the alternate winner will forfeit his or her right to claim the prize and the Florida Lottery will award the prize to a second alternate winner.  This process will continue until an alternate is contacted and the prize is claimed in accordance with subsection (5) below, or the Florida Lottery has exhausted the list of available alternates. If the Lottery is unable to contact an alternate, the grand prize will not be awarded.

    (5) How to Claim a Prize. To claim a grand prize in the Promotion, the grand prize winner must submit to the Florida Lottery the original valid non-winning WHEEL OF FORTUNE ticket bearing the entry number selected in the drawing. Without such ticket, the player will forfeit his or her right to claim a prize. The grand prize winner must submit the valid entry ticket along with a completed Winner Claim Form DOL-173-2, revised 09/13 or DOL-173-2S, revised 09/13 (Spanish version of the Winner Claim Form) and a copy of acceptable identification as set forth in the rule of the Florida Lottery governing payment of prizes. A copy of the current prize payment rule can be obtained from the Florida Lottery, Office of the General Counsel, 250 Marriott Drive, Tallahassee, Florida 32399-4011. The grand prize winner must also submit a completed Release and Authorization form DOL-474, effective 8/13, or Spanish Release and Authorization form DOL-474S, effective 8/13. Forms DOL-173-2, DOL-173-2S, DOL-474, and DOL-474S are hereby incorporated by reference and can be obtained from any Lottery office, or by writing to: Florida Lottery, Customer Service Division, 250 Marriott Drive, Tallahassee, Florida 32399-4016.  Forms DOL-173-2 and DOL-173-2S can also be obtained from the Lottery’s website, flalottery.com. The required claim documentation must be received by the Florida Lottery no later than two weeks after the winner is notified by the Florida Lottery that he or she is a winner. If the Florida Lottery has not received the required documentation from the grand prize winner by the fourteenth day after notification, the winner will forfeit his or her right to claim the prize, and the Florida Lottery will award the prize to an alternate winner in accordance with subsection (3) and paragraph (4)(b), above.

    (6) Award of Grand Prizes.

    (a) Upon the Florida Lottery’s receipt of a grand prize winner’s required documentation, the Florida Lottery will award a prize of a Grand Prize VIP Trip Package (“Trip Package’’). The Trip Package includes:

    1. Two round trip coach class tickets to Los Angeles, California, from any commercial service airport in the United States;

    2. Four nights’ hotel accommodations for two persons in the Hyatt Century City or an equivalent hotel in Los Angeles, California;

    3. Ground transportation to and from the Los Angeles airport, hotel and the lottery players-only non-broadcast WHEEL OF FORTUNE Game Show;

    4. Bus tour of Hollywood for two persons;

    5. Tour of Sony Studios for two persons;

    6. The chance to be selected as a contestant in a lottery players-only non-broadcast WHEEL OF FORTUNE Game Show;

    7. $1,000 in cash; and

    8. Payment of federal income tax withholding ($2,133.33 for a U.S. citizen) on the total retail value of the Trip Package elements described in subparagraphs 1. through 7., above ($6,400).

    The Florida Lottery will mail the grand prize winner a check for the cash portion of the prize and a certificate describing the prize along with the necessary information to make travel arrangements and fulfill the trip. The total value of a grand prize is $8,533.33.

    (b) The Trip Package does not include mileage, insurance, gratuities, meals, parking fees, departure fees, excess baggage fees, alcoholic beverages, and any items not expressly specified as being part of the Trip Package, including personal expenses such as room service, telephone calls, valet service, laundry and the like, as well as revision or cancellation fees which may be charged by the hotel, airlines or other suppliers.

    (c) If approved by the fulfillment company, IGT Global Solutions Corporation (“IGT”), and the Florida Lottery, a grand prize winner may appoint a proxy to take the trip and participate in the non-broadcast WHEEL OF FORTUNE Game Show on the grand prize winner’s behalf. A Proxy Appointment and Acknowledgement form DOL-491, Effective 3/16, should be completed by the appropriate parties prior to travel arrangements being made. Form DOL-491 is hereby incorporated by reference and can be obtained from any Lottery office, or by writing to: Florida Lottery, Customer Service Division, 250 Marriott Drive, Tallahassee, Florida 32399-4016. If a proxy is appointed, the grand prize winner will receive the cash portion of the Trip Package and all prizes won by the proxy in the non-broadcast WHEEL OF FORTUNE Game Show and shall be taxable on any prizes won by the proxy.  If the Lottery and IGT agree to appointment of a proxy by a grand prize winner after travel arrangements have been made, the grand prize winner shall immediately complete a Proxy Appointment and Acknowledgement form and be responsible for paying all additional charges imposed.

    (d) Unless otherwise specified by the Florida Lottery, if a grand prize winner or proxy or their guest fails to arrive for the scheduled flight or fails to check into the hotel, the grand prize winner or proxy or guest shall forfeit the flight and/or hotel portion of the Trip Package and shall be responsible for making any alternative arrangements.

    (e) In the event that a grand prize winner is unable to attend the trip due to unforeseen circumstances, and there is insufficient time to appoint a proxy to take the trip in his or her stead as described in paragraph (6)(c) above, the balance of the Trip Package shall be forfeited; however, an IGT representative shall fill in as a proxy to participate in the non-broadcast WHEEL OF FORTUNE Game Show for the grand prize winner. The grand prize winner shall receive and be taxable on any prizes won by the proxy.

    (7) Lottery Players-Only Non-broadcast WHEEL OF FORTUNE Game Show (or “Game Show”).

    The Trip Package includes a chance for the grand prize winner to be selected as a contestant in a lottery players-only non-broadcast version of the WHEEL OF FORTUNE Game Show (“Game Show Contestant”) at Sony Pictures Studios to be attended by the grand prize winner and his or her guest. The Game Show is anticipated to take place in January 2017.

    (a) During the Game Show, each selected Game Show Contestant shall play the WHEEL OF FORTUNE game and have the chance to win up to $1,000,000.00 in prizes.  Grand prize winners not selected as Game Show Contestants will be randomly assigned to teams which may include winners from other state lotteries (“Game Show Team”) with each Game Show Contestant named as the team captain.  Each Game Show Team will win the equivalent cash prize as their team captain.  Team prizes will be split evenly among the Game Show Team members. Only cash prizes will be awarded on the Game Show.

    (b) Each grand prize winner, regardless of whether or not they are selected as a Game Show Contestant, will spin the WHEEL OF FORTUNE wheel (the “Wheel Spin”) one time to win a cash prize ranging from $300 up to $2,500.

    (c) If a grand prize winner or proxy is unable to attend or fails to arrive at the studio for his or her scheduled participation in the Game Show, the Grand Prize winner forfeits the right to personally participate in the Game Show and an IGT representative shall fill in as a proxy for the grand prize winner in the Game Show. Any cash prize(s) won by any such proxy shall be awarded to the grand prize winner on whose behalf the proxy was appointed. The grand prize winner shall be taxable on any prizes won by the proxy.

    (d) The grand prize winner or the grand prize winner’s proxy must be 18 years of age or older to participate in the Game Show.

    (8) Taxes.

    (a)  The Florida Lottery will pay applicable Federal income tax withholding on the retail value of a grand prize ($6,400). The reportable taxable value of a grand prize includes the retail value of the prize and the value of the federal income tax withholding paid by the Lottery. The estimated reportable taxable value of a grand prize for a U.S. citizen is $8,533.33.

    (b) IGT shall be responsible for paying all cash prizes won by the participants in the Game Show and the Wheel Spin, except as provided in subsection (9) below.  Otherwise, cash payments to winners will be made directly by IGT within approximately fifteen business days of the Game Show, less applicable federal income tax withholding. IGT shall remit taxes on behalf of the grand prize winner and issue the applicable tax forms to the grand prize winner along with any other applicable documentation.

    (c) Except as specifically mentioned herein, payment of all federal, state and/or local taxes or other fees on the prizes won in conjunction with a winner’s participation in the WHEEL OF FORTUNE® Big Money Second Chance Promotion will be the responsibility of the winner.

    (9) State Owed Debt. Each grand prize winner will be analyzed twice for state owed debt. The first analysis will occur at the time the Trip Package is claimed. If the winner of the Trip Package is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with Section 24.115, Florida Statutes.  If the debt is an amount of less than the cash portion of the Trip Package prize ($1,000), the non-cash portion of the Trip Package and the cash portion of the Trip Package less the amount owed shall be awarded. If the winner is identified as owing such a debt in an amount greater than the cash portion of the Trip Package, the winner’s entire cash portion of the Trip Package will be applied toward the outstanding debt as provided in Section 24.115, Florida Statutes, and the winner will receive the remaining non-cash portion of the Trip Package. The second analysis will occur after the Game Show has concluded and the grand prize winner’s Game Show winnings have been determined. If the winner is identified as owing an outstanding debt to a state agency or child support collected through a court, the debt will be collected in accordance with Section 24.115, Florida Statutes, and the remaining winnings, if any, will be paid to the winner by the Florida Lottery.

    (10) Other Restrictions and Provisions.

    (a) The grand prize winner, grand prize winner’s proxy or guest must have a valid credit card or shall be required to post cash deposits for incidentals in order to check into the hotel, unless other arrangements are made directly with the hotel.  At least one member of the party must be at least eighteen years old in order to check into the hotel.

    (b) The minimum age requirement for any audience member at the Game Show is eight years.

    (c) The grand prize winner is solely responsible for the actions of the guest who accompanies the prizewinner to the Game Show. If a grand prize winner’s guest is a minor, the grand prize winner must either be the parent or legal guardian of such guest, or must present a notarized, written consent from the minor’s parent or legal guardian for the minor guest to accompany the grand prize winner on the trip.

    (d) All prizes are subject to the provisions of Chapter 24, Florida Statutes, and rules promulgated thereunder.

    (e) Players must be at least 18 years of age. Persons prohibited by Section 24.116, Florida Statutes, from purchasing a Florida Lottery ticket are not eligible to play.

    (f) The right to claim a prize cannot be assigned to another person or entity.

    (g) By entering the WHEEL OF FORTUNE® BIG MONEY Second Chance Promotion, a player gives his or her permission for the Florida Lottery to provide the player’s address and telephone number to IGT for prize fulfillment purposes.

    (h) A player entering the WHEEL OF FORTUNE® BIG MONEY Second Chance Promotion is deemed to have granted permission for the Florida Lottery to photograph and/or videotape and record the prizewinner with or without prior notification and to use the name, photograph, videotape, and/or recording of the prizewinner for advertising or publicity purposes without additional compensation.

    (i) WHEEL OF FORTUNE® BIG MONEY second chance drawings shall be public, held in Tallahassee, Florida, and witnessed by an accountant employed by an independent certified public accounting firm. The results of each drawing will be available after the drawing on the Florida Lottery’s website at flalottery.com.
