NOTICE OF CHANGE

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 39, No. 65, April 3, 2013 issue of the Florida Administrative Register.

In addition to the rule edits reflected in the Notice of Change published in Vol. 39, No. 95, May 15, 2013, issue of the Florida Administrative Register, Form DSBD 153-2 as incorporated by reference, has been changed. Form DSBD 153-2 has been revised and the following language has been removed “The specification of forms under this subsection does not preclude the use of other forms.”
