    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 41 No. 88, May 6, 2015 issue of the Florida Administrative Register.

    These changes are in response to written comments received from the staff of the Joint Administrative Procedures Committee:

     

    12-18.001 Authorization for Compensation.

    The Department withdraws proposed subparagraph (1)(b)15., F.A.C.,which read “15. Prepaid wireless E911 fees;” and renumbers subparagraphs 16. through 20.

     

    12-18.004 Submission of Information and Claims for Compensation.

    The Department revises Form DR-55, Application for Compensation for Tax Information, as incorporated by paragraph (3)(a), to remove the addition of “prepaid wireless E911 fees” on page 2.

     

    12-18.008 Compensation for Vending Machine Violations.

    The Department adds a new subparagraph (5)(A)5. to read as follows:

    5. Social security numbers are used by the Florida Department of Revenue as unique identifiers for the administration of Florida’s taxes. Social security numbers obtained for tax administration purposes are confidential under Sections 213.053 and 119.071, F.S., and are not subject to disclosure as public records. Collection of an individual’s social security number is authorized under state and federal law. Visit the Department’s Internet site at www.myflorida.com/dor and select “Privacy Notice” for more information regarding the state and federal law governing the collection, use, or release of social security numbers, including authorized exceptions.
