NOTICE IS HEREBY GIVEN that on April 26, 2016, the Agency for Health Care Administration, received a petition for Waiver of paragraph 59A-4.203(1)(a), Florida Administrative Code, from the Petitioner, Consolidated Group of Tampa, Inc. d/b/a Ybor City Healthcare & Rehabilitation Center. The Petition requests waiver of Florida Administrative Code Rule 59A-4.203(1)(a), requiring a report of audited financial statements specifying an unqualified opinion.
