NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 44 No. 62, March 29, 2018 issue of the Florida Administrative Register.
Summaries of Substantive Changes to Form DBPR-DDC-220 are as follows:
Form DBPR-DDC-220:
On page 8 of 12, the department has revised Form DBPR-DDC-220 to amend questions 2. and 2.a. in Section VI of the application to request applicant’s licensure from any jurisdiction, in addition to other state licenses, and to specifically request whether applicant is licensed as a third party logistics provider from any other state or jurisdiction, in addition to licensure as a manufacturer or wholesaler of prescription drugs.
On page 8 of 12, the department has revised Form DBPR-DDC-220 to amend questions 3. and 3.a. in Section VI of the application to request applicant’s licensure from any jurisdiction, in addition to other state licenses.
