    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 47 No. 68, April 8, 2021 issue of the Florida Administrative Register.

    The changes are in response to written comments submitted by the staff of the Joint Administrative Procedures Committee and discussion and subsequent vote by the board at a public meeting held May 21, 2021.  The changes are as follows:

    64B2-18.005 Certification of Supervising Physicians.

    (1) After initial certification and establishment of an initial work arrangement, any certified chiropractic physician’s assistant or Any chiropractic physician desiring to supervise, employ or work with a certified chiropractic physician’s assistant other than as initially established, shall make application to the Board on board approved form DH-MQA 1149, (Rev 06/20), Modifying Supervision for Certified Chiropractic Physician’s Assistant Application, which is hereby incorporated by reference, and may be obtained from http://www.flrules.org/Gateway/reference.asp?No=Ref-       , or the Board of Chiropractic Medicine, 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257, accompanied by the required fees.

    (2) through (4) No change.
