NOTICE IS HEREBY GIVEN that on May 29, 2024, the Board of Opticianry, received a petition for variance and waiver filed by Carlos Perez. Petitioner seeks a variance or waiver of Rule 64B12-16.004, F.A.C., which requires if an apprentice terminates apprenticeship with a sponsorship with a sponsor upon completion of the program hours, or the sponsor is no longer providing training to an apprentice, the apprentice shall submit to the Department within 30 days from the date of the termination the completed Apprenticeship Sponsor Report Form DH-MQA 1063.
