    64B3-6.002: Documentation for Licensure

    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 43 No. 79, April 24, 2017 issue of the Florida Administrative Register.

    This change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee and from a vote by the Board at a duly noticed public meeting held on June 2, 2017.  The rule shall now read as follows:

    64B3-6.002 Documentation for Licensure.

    The following is a list of acceptable documents which shall be submitted to the Board as appropriate for the type of license sought to show eligibility for the license:

    (1) No change.

    (4) A copy of certificate of attendance or documentation of training required pursuant to Chapters 64B3-3 and 64B3-4, F.A.C., and continuing education required pursuant to Chapter 64B3-11, F.A.C., including one (1) hour HIV/AIDS continuing education or affidavit stating that the course will be completed within six (6) months of licensure, An applicant for initial licensure who has not taken an acceptable HIV/AIDS course at the time of licensure may submit an affidavit showing good cause and shall be allowed six (6) months to complete the course.

    (5) through (8) No change.
