NOTICE IS HEREBY GIVEN that on August 3, 2012, the Division of Emergency Management, Bureau of Preparedness, Technological Hazards Section, received a petition for University Hospital & Medical Center. Petitioner seeks a waiver of the requirements of Sections 252.85(4), (5), Florida Statutes, adopted by Rules 9G-14.004 and 9G-14.005, Florida Administrative Code, that requires payment of late fees and penalties which pose a significant economic/financial hardship. Petitioner has satisfied the reporting requirements.
