PURPOSE AND EFFECT: The proposed rule incorporates two updated Department of Health forms regarding Hearing Aid Specialists training program. Primarily the changes to the forms are necessary to clarify that program only applies to prescription hearing aids and does not apply to over-the-counter hearing aids. There are also other minor changes to the forms such as updating the existing email address.

SUBJECT AREA TO BE ADDRESSED: The rule addresses the applications for Hearing Aid Specialists Application for Examination, Form 1155 and Training Program Continuation Request, Form DH-MQA 1160.
