PURPOSE AND EFFECT: The Board proposes to amend rule language to include interns and residents, and to advise licensee of the requirement to update information.
SUMMARY: The rule is being amended to include licensees current personal information for license.

Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

THE FULL TEXT OF THE PROPOSED RULE IS:
64B14-1.004 Address of Record and Place of Practice.
(1) Each person holding a license or registration issued pursuant to Chapter 468, Part XIV, F.S., must maintain on file with the Board of Orthotists and Prosthetists the current address at which any notice required by law may be served by the Board or its agent. Within 30 days of changing this address, whether or not within this state, the licensee shall change his or her address at the Boards website [www.doh.state.fl.us/mqa/OrthPros/index.html] or notify the Board by e-mail or in writing of the new address at which the licensee may be served with notices or other documents.
(2) Each person holding a license or registration issued pursuant to Chapter 468, Part XIV, F.S., must maintain on file with the Board of Orthotists and Prosthetists the current primary place of practice, to include the practice name, address, and phone number. Within 30 days of changing this address, whether or not within this state, the licensee or registrant shall notify the Board by e-mail or in writing of the new practice information.
