    NOTICE OF CHANGE

    Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 39, No. 112, June 10, 2013 issue of the Florida Administrative Register.

    The following changes have been made to the materials incorporated within this section:

    Forms DBPR ELC 1-5: “(Optional)” was added after all email requests on the application. The instructions inform the applicant that email addresses are public record.

    Forms DBPR ELC 2-5: The rule “61G7-.10.002” was removed from the footer of the applications.

    Forms DBPR ELC 2, page 13 and DBPR ELC 3, page 14: “Contract Provisions Checklist” reference to Rule 61G7-12.001, FAC. was added and the column heading “Statute Provision” was changed to “Statute/ Rule Provision”

    The effective date on all the forms is August 2013, the month/year the final version of the form is completed.
