PURPOSE AND EFFECT: To incorporate by reference these three new forms: State University System Optional Retirement Program Application for Refund of Voluntary Employee Contributions Only form, State University System Optional Retirement Program Application for Retirement and Retirement Distribution form, and State University System Optional Retirement Program Application for Required Minimum Distribution Authorization form. To update the Optional Retirement Program Participation section under rule 60U-1, F.A.C., by creating definitions for the terms “FRS,” “employer,” “mandatory participant,” “participant,” “provider company,” SUSORP,” and “SUSORP election window.”, update contact information, create sections on program administration, employer and employee contributions, reemployment provisions and renewed membership, and forfeiture of benefits.
