NOTICE IS HEREBY GIVEN that on August 30, 2018, the Board of Dentistry, received a petition for variance or waiver filed by J. Terry Alford, seeking a variance or waiver of subsection 64B5-14.0032(4), Florida Administrative Code, which provides the provisions regarding Non-Sedation Permit Holders for the treatment of dental patients in an outpatient dental office setting where a physician anesthesiologist is performing the sedation services.
