The proposed rule amendments add definitions; change licensure requirements; specific definitions regarding reporting requirements; changes in reporting requirements including a new reporting form incorporated by reference; development of outcome measures; adoption of national initiatives developed by the National Hospice and Palliative Care Organization; and additional language, clarification of terms, and updating the Health Care Advance Directives form incorporated by reference. TWO VERSIONS OF DOEA FORM H-002, INCORPORATED BY REFERENCE IN RULE 58A-2.005, F.A.C., WILL BE REVIEWED: ONE VERSION INCLUDES IDENTIFYING INFORMATION AND OUTCOME MEASURES; THE SECOND VERSION INCLUDES IDENTIFYING INFORMATION, OUTCOME MEASURES AND REPORTING INFORMATION REQUIRED IN RULE 58A-2.012, F.A.C.
