The purpose of this rule amendment is to incorporate by reference update September 2009 to the Florida Medicaid Provider Reimbursement Handbook, CMS-1500. The handbook update requires Medicaid durable medical equipment and medical supplies providers to enter the prescribing physician’s name and National Provider Identifier on the claim and Medicaid home health providers to enter the ordering physician’s name and National Provider Identifier on the claim. The handbook update also contains policy clarifications. The effect will be to incorporate by reference in rule update September 2009 to the Florida Medicaid Provider Reimbursement Handbook, CMS-1500.
