Technical changes only have been made to this form due to an agency reorganization resulting in agency division name changes. The following technical changes have been made to the form: The Form number shown at the bottom of each page has been changed to replace the acronym RAD to TEL; In paragraph 4, on the second page of the Form, Form number PSC/ADM 124 has been changed to PSC/AIT 124; In paragraph 7, “Division of Regulatory Analysis” has been changed to “Office of Telecommunications” and the word “division” changed to “office.”.   3/3/2014
