There are three technical changes to the Participation Agreement: 1) since the Participation Agreement is applicable to all program participants, the date at the top was removed to prevent confusion; 2) the text was formatted (justified text and added line between columns); and 3) added "adopted by reference in Rule 19B-16.003, F.A.C." to the bottom for consistency with other program forms.   9/17/2013
