A Variance from Fla. Admin. Code Ann. r. 69O-149.005(14)(b) from Advantica Insurance Company was received by the Office of Insurance Regulation on September 21, 2017. The petition requests that the Office grant a variance from the multiple year rate guarantee or rating cap provision that may not exceed 24 months. Petitioner requests that the time frame be extended at least forty-eight months or such longer period as the Office permits with respect to group vision and dental insurance policies insuring groups of 51 or more persons.

On September 21, 2017, the District's Governing Board issued SFWMD Order No. 2017-065-DAO-ROW to New Cingular Wireless PSC, LLC (Application No. 17-0531-2). The petition for waiver was received by the District on May 31, 2017. Notice of receipt of the petition requesting the waiver was published in the Florida Administrative Weekly, Vol. 43, No. 108, on June 5, 2017. No public comment was received. This Order provides a waiver of the District's criteria for utilization of the District right of way for access. Specifically, the Order grants a waiver from Rule 40E-6.221(3)(j), Fla. Admin. Code, and the Basis of Review for Use or Occupancy of the Works or Lands of the District, incorporated by reference in Rules 40E-6.091(1), Fla. Admin. Code, which prohibits use of the Works or Lands of the District for permanent access to private property. Generally, the Order sets forth the basis of the Governing Board decision to grant the waiver as follows: 1) the proposed use will not interfere with the District's current ability to perform necessary construction, alteration, operation, and routine maintenance activities; and 2) the Order granting a waiver from the subject rule is based upon a substantial hardship.

The Florida Department of Transportation (FDOT) District Three hereby announces a Public Hearing to which all persons are invited. The hearing will be held in three sessions, October 10, 2017 in the FDOT District Three Design Conference Room, 1074 Highway 90, Chipley, FL 32428, and broadcast live via webinar. In addition, the webinars will broadcast at the following FDOT locations:
• FDOT Midway Operations Center, 17 Commerce Boulevard, Midway, Florida 32343
• FDOT Milton Operations Center, 6025 Old Bagdad Highway, Milton, Florida 32583
Project information for the various counties will be covered in accordance with the following session schedule:

A public information meeting is being held regarding the resurfacing project on State Road (S.R.) 406/Garden Street from west of Forrell Avenue to Washington Avenue, in Titusville, Brevard County.
The proposed improvements include the milling and resurfacing of all the travel lanes and auxiliary lanes along SR 406, providing bicycle accommodations in both directions by reducing lane widths through the use of pavement markings, and updating pedestrian ramps and signal features to comply with Americans With Disabilities Act (ADA) requirements.
The public information meeting will be 5-7 p.m. Tuesday, October 3, 2017, in the Council Chambers of Titusville City Hall, 555 South Washington Avenue, Titusville, FL 32796. Project information will be on display, and a brief presentation will run periodically throughout the meeting. Project representatives will be available to answer questions.

The Florida Department of Transportation (FDOT), District One, will hold a public hearing about median modifications and pedestrian safety improvements. These modifications are part of a resurfacing project on SR 70 from Orange Avenue to Turner Avenue, DeSoto County, FL.
FDOT did pedestrian counts in 2015 and determined for safety reasons the mid-block pedestrian crossings are needed. As part of this resurfacing project, FDOT is designing mid-block pedestrian crossings between North 10th Avenue and 11th Avenue, and just east of 12th Avenue. The mid-block crossings will provide designated areas for pedestrians to cross SR 70.
