15A-5.008. Deficits of Memory or Judgment

1Applicants 2or licensed drivers 5who suffer from deficits of memory or judgment must present a 16Medical Report, HSMV 72423, effective 07/18, incorporated by reference in Rule 2715A-5.004, 28F.A.C., 29from their treating physician indicating their 35physical and mental ability to safely operate a motor vehicle. 45A copy of the form is available on the Department’s website.
