40E-3. Water Wells |
1(1) The water well contractor shall complete and submit 10DEP Form 62-532.900(2), 13State of Florida Well Completion Report, (October 7, 2010) (23https://www.flrules.org/Gateway/reference.asp?No=Ref-0057825), 26incorporated by reference herein, 30to the District and delegated agency, as identified in Rule 4040E-3.035, 41F.A.C., 42for the construction, repair, or abandonment of all wells, regardless of whether a permit is required under Rules 6040E-3.041, 6140E-3.101, 62and 6340E-3.600, 64F.A.C. Well completion reports shall be filed with the District and delegated agency, as identified in Rule 8140E-3.035, 82F.A.C., 83within 30 days of completion of the work, 91using DEP Form 9462-532.900(2)95. 96Copies of DEP Form 10062-532.900(2) 101are available at no cost by contacting the South Florida Water Management District Clerk, 3301 Gun Club Road, West Palm Beach, FL 33406, (800)432-2045, ext. 6436 or (561)682-6436, and the delegated permitting authorities’ offices, 135as identified in Rule 13940E-3.035, 140F.A.C141. 142Well completion reports for sites controlled by Chapter 62-761, F.A.C., Underground Storage Tank Systems, may include all monitoring wells for the same site on a single form.
169(2) The water well contractor shall keep or cause to be kept by a person in his employ an accurate field log of all well construction, repair, or abandonment activities performed under each permit. Such logs shall be available for inspection at the site during all times when work is in progress.
221(3) If no work is performed or if the well is not completed, a report shall be filed within 30 days of the expiration of the permit stating that no well construction was performed under the permit or outlining the status of the incomplete well.
266(4) For water test wells, a report on the test results shall be submitted to the District within 30 days of completion of the testing. The report shall also include a request and a proposed schedule to either abandon the water test well or convert the water test well to a production well or monitoring well.
