    952(b) 953I954f the 956Lottery is unable to 960have personal 962contact 963with 964the 965grand 966prize winner 968within 969two weeks 971of the date of the drawing, the winner will forfeit his or her right to claim the prize and the Lottery will award the prize to 997the first 999alternate winner1001. 1002If the Lottery is unable to 1008have personal 1010contact 1011with 1012the 1013first 1014alternate winner within 1017two weeks 1019of the date of 1023award of the prize, 1027the alternate winner will forfeit his or her right to claim the 1039grand 1040prize and the Lottery will award the prize to a second alternate winner. This process will continue until an alternate 1060is contacted 1062or the Lottery has exhausted the list of available alternates1072. If the Lottery is unable to contact an alternate and the list of available alternates is exhausted, 1090the prize will not be awarded.

     

     

    1096(c) 1097If the Florida Lottery is unable to have personal contact with a second or third prize winner within thirty days of the date of the drawing 1123to confirm the winner’s mailing and email addresses, 1131the winner will forfeit his or her right to claim the prize and the prize will not be awarded. 1150A second prize winner who forfeits his or her right to claim a second prize will also forfeit his or her right to serve as an alternate for a grand prize.

Without such ticket, the winner will forfeit his or her right to claim a prize.

    1272The winner must submit the valid entry ticket along with 1282a 1283completed Winner Claim Form DOL 1288173-2, revised 0129191292/112933, 1294or Spanish Winner Claim Form DOL 1300173-2S, revised 1302091303/113043, 1305and 1306a 1307copy of acceptable 1310identification 1311as 1312set forth in the rule of the Florida Lottery governing payment of prizes. Therefore, grand prize alternate winners should retain the original non-winning 1557$25,000 DREAM MAKEOVER 1561Scratch-Off ticket until notified by the Florida Lottery that the grand prize has been fulfilled. 1576A 1577grand prize 1579alternate 1580winner 1581who has 1583claimed 1584a second prize at the time of notification 1592that he or she is the alternate winner of the grand prize 1604will receive the difference in 1609gift cards 1611between the grand and second prizes.

    1793(179481795) 1796Award of Second Prizes. 1800Upon making personal contact with a second prize winner, 1809the Florida Lottery will award a prize of a $500 The Home Depot gift card. 1824A second prize 1827winner 1828shall 1829choose 1830via an email 1833sent by the Florida Lottery, 1838one of two methods 1842for receiving the gift card 1847prize1848: 1849U.S. 1850mail (1852physical gift card) or electronically 1857(electronic gift card)1860. 1861The Florida Lottery will notify the fulfillment company of the name and contact information of the winner and the winner’s delivery preference1883. 1884Based on the winner’s delivery preference, 1890the fulfillment company 1893will 1894either 1895ship 1896the physical gift card 1900or email the 1903electronic gift card 1906within approximately ten business days after notification.

    1913(a) 1914Physical Gif1916t Card1918. 1919A physical 1921gift card 1923will be shipped via U1928.1929S1930. 1931m1932ail with tracking confirmation. In the event a tracking number does not have a delivery confirmation status, 1949the fulfillment company 1952shall attempt to contact the 1957prize1958winner 1959weekly, for two consecutive 1963weeks 1964via 1965tele1966phone and/or email, 1969to determine if the delivery was successful. If the 1978prize1979winner confirms 1981that 1982the prize 1984is 1985‘lost’ and the 1988delivery address is confirmed to be the address provided in the winner’s 2000registration, 2001a replacement 2003gift card 2005with the full value 2009will be sent to the 2014prize2015winner at no additional charge. 2020Neither the Florida Lottery nor 2025the fulfillment company 2028will be responsible for lost or stolen prizes after delivery confirmation of the gift cards. 2043In the event that a prize is returned as undeliverable, 2053the fulfillment company 2056will make all reasonable 2060efforts to locate the 2064prize 2065winner. 2066In the event that 2070the 2071fulfillment 2072company 2073is unable to locate the 2078prize2079winner within thirty calendar days of return of the prize, 2089the prize will not be awarded.

The e2167lectronic gift card 2170can be printed or stored on a mobile device for use. 2181If a2183n electronic gift card 2187is undeliverable and the 2191Lottery and the 2194fulfillment company 2196are 2197unable to c2200ontact the 2202prize2203winner after a reasonable attempt to do so, the prize will not be awarded.

    2217(9) Award of Third Prizes. Upon making personal contact with a third prize winner, the Florida Lottery will award a prize of a $100 The Home Depot gift card. 2246The Florida Lottery will notify the fulfillment company of the name and contact information of the 2262prize2263winner and the fulfillment company will email the 2271prizewinner the 2273gift card within approximately ten business days after notification. 2282All third 2284prize 2285gift cards shall be delivered electronically 2291as described in paragraph (8)(b) above.
