900(2) 901How to Play 904CASH4LIFE905.
906(a) In 908CASH4LIFE, 909players select five numbers from a field of one through sixty 920and one 922number 923from a separate field of one through 930four 931for each Play.
934(b) Players may make their 939CASH4LIFE 940ticket selections by marking a play slip or by telling the retailer their desired selections. There are five panels on a play slip, each containing an upper play area and a lower play area. Each panel played will cost $2.00 per Play, per drawing. Players may mark their desired numbers on the play slip by selecting six numbers (five in the upper play area and one in the lower play area) from each panel played. Players may also mark the “QP” (Quick Pick) box located at the bottom of each play area for the terminal to randomly select any or all of the six numbers from either or both play areas. 1051A “Void” box is also located at the bottom of each panel and should be marked by the player if an error was made in his or her selections in a panel. For each panel played, the first five of the six numbers appearing in a single horizontal row on a 1102CASH4LIFE 1103ticket shall be the numbers selected from the upper play area of the play slip, and the last number shall be the 1125Cash Ball 1127N1128umber selected from the lower play area of the play slip.
1139(c) Players may mark the $10 “Quick Picks for next draw” box to receive one ticket with 1156five 1157sets of six randomly selected numbers for the next CASH4LIFE drawing, or may mark the $20 “Quick Picks for next draw” box to receive one ticket with 1184ten 1185sets of six randomly selected numbers for the next CASH4LIFE drawing. Players may mark Quick Picks in addition to panel plays.
1206(1207d1208) Players must use only blue or black ink or pencil for making selections. 1222The use of mechanical, electronic, computer generated or any other non-manual method of marking Play Slips is prohibited. 1240Play slips may be processed through a Florida Lottery full service vending machine or processed by a 1257Florida Lottery 1259retailer to obtain a ticket. Retailers also are authorized to manually enter numbers selected by a player.
1276(1277e1278) Advance Play. Players may play up 1285to twenty-six 1287consecutive 1288CASH4LIFE 1289D1290rawings by using the “advance play” feature. To use the advance play feature, players may either mark the number of drawings desired in the “Advance Play” section of a play slip or tell the retailer their desired number of consecutive advance drawings. The number of consecutive drawings marked will include the next available drawing and will apply to each panel (A-E) played. In the event that a planned change in the 1361CASH4LIFE 1362game requires that the number of advance plays available for purchase be reduced to zero before implementation of the change, an advance play countdown schedule will be posted on the 1392Florida 1393Lottery’s website.
1395(3) 1396CASH4LIFE Drawings.
1398(a) CASH4LIFE 1400D1401rawings to determine the 1405CASH4LIFE Winning Numbers 1408shall be conducted 1411at the studios of 1415New Jersey Lottery Headquarters (or elsewhere as determined by the Party Lotteries) on Monday and Thursday at approximately 9:00 p.m. 1435(1436ET1437); 1438however, the day, time or location of Drawings may be modified as determined by the Directors and publicly announced by the Party Lotteries. Substitute Drawings may be held at the discretion of the state holding the Drawing. All drawings shall be public and witnessed by an independent 1485certified public 1487accounting firm.
1489(b) 1490Manner of 1492C1493onducting 1494D1495rawings1496. 1497The drawing officials will select, at random, with the aid of mechanical devices or any other selection methodology as authorized by the Party Lotteries, the CASH4LIFE Winning Numbers.
1525(1526c1527) 1528Neither the Party Lotteries 1532n1533or t1535he Florida Lottery shall be responsible for incorrect circulation, publication or broadcast of official Winning Numbers.
1551(4) 1552Determination of Prize Winners1556.
1557In order for a ticket to be a winning ticket, numbers appearing in a single horizontal row 1574(the Player Number Sequence and the Cash Ball Number) 1583on the ticket must match the official 1590CASH4LIFE 1591Winning Numbers in any order for the drawing date for which the ticket was purchased, in one of the following combinations:

2176(a) 2177Top Prize 2179– For any dra2183wing, 2184if the total liability exceeds $7,2190000,000 2192(2193there is more than one 2198Top Prize 2200Winning 2201Play2202), 2203the 2204Top Prize 2206amount for each 2209Winning Play 2211shall 2212be $7,2214000,000 2216divided by the number of 2221Winning Plays on Winning Tickets 2226sold.
2227(b) 2228Second Prize 2230– For any drawing, 2234if the total liability exceeds $5,2240000,000 2242(there are more than five Second Prize 2249Winning 2250Plays2251), 2252the 2253Second Prize 2255amount for each 2258Winning Play 2260shall be 2262$5,2263000,000 2265divided by the number of 2270Winning Plays on Winning Tickets 2275sold2276.
2277(c) 2278In the event that any single drawing should produce more than seven 2290Top Prize 2292Winning Plays 2294and at least one Second Prize 2300Winning Play, 2302both the 2304Top Prize 2306and 2307Second 2308Prize 2309will 2310become pari-mutuel prizes. The total prize pool to be allocated equally among the total number of 2326Winning Plays 2328in this scenario will total $7,2334000,000 plus the lesser of $5,000,000 2343or $1,2345000,000 2347multiplied by the number of 2352Second Prize 2354Winning Plays2356. Each 2358Top Prize and Second Prize 2363Winning Play 2365will share the 2368defined prize pool equally 2372for the cash value, and the Annuity Option will be calculated 2383as set forth in 2387paragraphs 2388(9)(b) and 2390(10)(b)2391.
2392(239392394) 2395Top 2396P2397rize 2398P2399ayment2400. 2401In any single drawing, a 2406Top 2407Prize shall be divided equally among the number of 2416Top 2417Prize 2418Winning Plays2420. 2421Except as provided 2424herein, 2425a 2426Top 2427Prize shall be paid either as an annuity 2435(2436“Annuity Option”) 2438or lump sum cash payment 2443(“Cash Option”)2445. 2446Top 2447Prize winners have sixty days after the winning draw date to choose between the two payment options. Once the 2466Top 2467Prize winner signs the Winner Claim Form, files a claim and exercises the winner2481’2482s chosen option, the election of that option shall be final and cannot be revoked, withdrawn or otherwise changed. 2501If the Top Prize winner does not elect the Cash Option within sixty days after the winning draw date, the Annuity Option will be applied, except as provided in 2530subparagraph 2531(9)(2532b2533)25342., 2535below. 2536Shares 2537of the 2539Top 2540Prize shall be determined as follows:

2795(2796a2797) 2798Except as provided 2801herein, 2802a Second Prize shall be paid either as an annuity (“Annuity Option”) or lump sum cash payment 2819(“Cash Option”)2821. 2822Second Prize winners have sixty days after the winning draw date to choose between the two payment options. Once the 2842Second 2843Prize winner signs the Winner Claim Form, files a claim and exercises the winner2857’2858s chosen option, the election of that option shall be final and cannot be revoked, withdrawn or otherwise changed. 2877If the Second Prize winner does not elect the Cash Option within sixty days after the winning draw date, the Annuity Option will be applied, 2902except as provided in 2906paragraph 2907(10)(b), 2908below. 2909For each Second Prize 2913Winning 2914Play, the annuitized prize value will be $1,000 per week for 2926l2927ife, paid in annual installments of $52,000, or 2936a one2938-time lump sum cash prize payment 2944of $1,000,000, 2948unless a liability limit 2952would be exceeded.

The 3001Annuity 3002P3003ayment amount shall be the pari-mutuel cash value of the 3013Winning Play 3015divided by $1,000,000 multiplied by $52,000. 3024If the liability limit is exceeded and 3031the annuity prize value of a Second Prize falls below $500 per week for 3045l3046ife, then all Second Prize 3051Winning 3052Plays will be paid in a single lump sum cash payment and no annuitized payment option will be available. The minimum Second Prize 3075value 3076shall not be less than any prize payout for the 3086T3087hird through the 3090N3091inth 3092P3093rize levels for the same 3098CASH4LIFE 3099Drawing.
3100(11) 3101Annual Payment Schedule. 3104The winner 3106of a Top 3109Prize 3110or Second 3112P3113rize who elects the Annity Option 3119will receive the first full annual installment as soon as reasonably possible after the prize claim is validated. All subsequent annual payments for prizes claimed by an individual will be paid in equal annual installments for the 3156Measuring 3157Life 3158of the winner, with a minimum of twenty years of payments. All subsequent annual payments for prizes claimed by a trust, corporation or other legal entity shall be paid in equal installments, such that the total payments (including the initial installment) equal twenty years of payments.

3257(1325843259) 3260Life Annuity Requirements 3263and Restrictions. 3265An 3266Annuitant will be paid their appropriate 3272Top 3273Prize share or Second Prize share on an annual basis for their Mea3286suring Life, or for the twenty 3292year term of the Guaranteed Annuity Portion whichever is longer. 3302At the end of the guaranteed twenty year payment period, 3312Annuitants 3313will be required to provide 3318required 3319evidence to the Lottery that they are still living 3328in order to receive any Deferred Annuity Portion Installments. 3337Annuitants 3338will be required to provide such evidence on an annual basis prior to receipt of their next scheduled payment. 3357In the event that an Annuitant dies within the twenty year Guaranteed Annuity Portion, any remaining prize payments in the Guaranteed Annuity Portion will be paid 3383as 3384set forth in the rule of the Florida Lottery governing payment of prizes. 3462(1346363464) 3465Taxes. 3466Federal withholding taxes shall be deducted from prizes 3474for which it is applicable 3479in accordance 3481with the Internal Revenue Code and Code of Federal Regulations3491.
3492(1349373494) 3495CASH4LIFE Rules and Prohibitions.
3499(a) By purchasing a CASH4LIFE ticket, a player agrees to comply with and abide by all rules of the Florida Lottery.
3520(b) 3521All liabilities for a CASH4LIFE prize are discharged upon payment of a prize claim. 3535All claims arising out of the CASH4LIFE game must be pursued only against the State of ticket purchase, and litigation, if any, shall only be maintained against the Party Lottery of the State of ticket purchase and within 3573the State of ticket purchase as the sole and ex3583clusive remedy of the claimant.
3588(3589c3590) 3591Florida CASH4LIFE prizes shall be claimed only through a Florida Lottery retailer (for prizes less than $600) or Lottery office beginning on the first business day following the drawing. 3620Top Prizes and Second Prizes must be claimed at Lottery 3630headquarters. The 3632Lottery is not authorized to accept claims or pay prizes for CASH4LIFE tickets purchased in other jurisdictions. 3649CASH4LIFE prize payments shall be made in accordance with the rule of the Florida Lottery governing payment of prizes. 3693(3694d3695) Subject to a retailer’s hours of operation and on-line system availability, CASH4LIFE lottery tickets are available for purchase daily between the hours 3718of 6:00 a.m. and 12:00 midnight 3724(3725ET3726)3727. Ticket sales for a specific CASH4LIFE 3734D3735rawing will close at 37398:30 3740p.m. 3741(3742ET3743), 3744on the night of the drawing. Any ticket sold after the close of game will 3759be printed with the next CASH4LIFE 3765D3766rawing date.
3768(3769e3770) CASH4LIFE tickets cannot be canceled.
